This is all I've managed to find so far, but it was at Edmonton in 1971:-
"Edmonton (Sep 26): Hulme 1st Revson 12th (started 11 laps late after bolt dropped down injection trumpet, sabotage suspected)"

From Pete Lyon's excellent CanAm book:
"On race morning, some mechanics discovered evidence of overnight tampering. One of Motschenbacher's brake lines appeared to have been pinched with pliers while Amon's Ferrari was missing a locking pin the mechanics were sure had been in place the night before."
