Does anyone have any information about the car, and why production wasn't started?

Are there any other examples of other companies building one-off/prototype F5000 cars?

I hope the prototype was in about 68 or 69. Otherwise the bodywork at least is way out of date. Looks like a tube frame and I doubt there is an engine in it. No exhausts at least.
Nota made a F2 car about that time I think?

There were several "one offs" built in the UK but I think they were home brewed so can hardly be called a manufacturer.

IIRC Modus built a F5000 for Tom Walkinshaw.

I don't know of any plans Nota ever had to build a F5000 car and this photo looks to me more like one of the ANF3 cars they built. It is simply not big enough to have been a F5000 particularly the wheels and tyres! The transmission looks a bit Renault like and at least one of the F3 cars was Renault powered.

Yes that's right Paul, I've got a shot of that car in an Autosportsman and it was never touted as a F5000.

When I saw the photo the first thing I looked at were the wheels and tyres , at best the car is running 6.0''fronts
and 8.0'' rears and as Paul stated that is not F5000 territory .

I can only come up with two owners of these F3 cars , i.e. Ian Cook [ Sydney version ] and Alf Lee.
