So folks, own up, who else has committed such a glaring error and did it cost you a championship? 

Julian Simon did it in 2009 in the 125cc, but did go on to win the title that year - ahead of Bradley Smith, but I can't think of anyone else at the top level who has done it though.

How about Jorge Lorenzo signing for HRC just as he'd got to grips with the Ducati.

Or Philip MaCallen missing his pit stop in the 1993 Junior TT

Daytona 1981 - I muddled the oil/petrol ratio up converting US pints to Imperial pints then to milliliters, luckily for me it was Martin Wimmers' bike that seized, not ours.

I believe the Parrish incident was true and concerning Martin Wimmer, I don't remember him coming back for more 'advice'.
