There have been several incidents in recent years of these. Mainly these happen in the first lap and often drivers are unaware of what has happened behind them.  There may be two or three cars side-by-side while SC information comes out and it's very difficult to distinguish who is ahead. Especially if neither driver doesn't know that there has been incident behind them.

While overtaking under yellows, especially under SC should be taken harshly, these are quite different to that. The penalties we usually see are 5 seconds or so. However, there could be other way to do it.

I say that especially when safety car is deployed on the first lap it should be easy to find the order after first or second sector and then put cars in that order during the SC period without any need to give penalties. Also it could make it easier to avoid further incidents if drivers react to the upcoming safety car at different times. Good examples are Viso's accident in Magny-Cours and Formula Renault 2.0 crash from Silverstone 2015.

I think in F1 and all other categories that have good flag-in-the-cockpit systems, the problem of drivers tripping over each other is reduced significantly. If you're relying solely on the SC boards and flags, it can be an issue if you're tucked in behind another car because he may see flags which you can't see because he's obstructing your view. I can't think of anything more that F1 could do about this, though.

In terms of determining the order, in V8 Supercars, if cars are side-by-side when the SC is called and
each driver thinks he was ahead, generally they seem to be able to resolve it by means of the teams asking race control to adjudicate. Generally, before the SC comes in, you'll hear the Race Director either confirming that the running order is correct, or reading out what he considers to be the correct running order based on where the cars were when the SC was called and telling the teams to sort themselves out. As long as the Race Director doesn't think anyone was intentionally cheating or being dangerous, there's no penalty beyond letting the other driver back past. Tim Schenken is quite good at his job, though, so who knows what would happen if Whiting tried to do this.

It does seem to me to be the right way to handle it if a pass is made very soon after the SC is called, though. Once you're committed to outbraking somebody and then the SC comes out when you're part way through the move, you're already braking as hard as you can and you can't stop yourself going past. Similarly, if you're going around the outside of somebody, you may not be able to see them or judge the exact moment when you move ahead. For me, actual penalties should only come into play if a driver is judged to have initiated a passing move after the SC has been cralled.
