Clay Regazzoni joined Ferrari and Ensign twice.
And while technically two different "Williams" teams, Jacques Laffite drove for both FWRC (1974-75) and WGPE (1983-84), with two periods at Ligier either side of his "second" Williams stint.
Laffite and Heikki Kovalainen seem to share the distinction of having driven for two separate teams with the same name, Heikki having driven for Enstone (as Kimi sub) and Team Tony when both were called "Lotus", while also having a prior full-time Enstone stint under the Renault name
EDIT: Thought of Ronnie Peterson (March & Lotus) too, and while not meeting the multiple teams criteria of the thread I always think any excuse to give Piercarlo Ghinzani's whopping four shifts at Osella an honourable mention.

He also reportedly almost re-signed for Jordan at one point didn't he? Didn't they try to re-sign him as a one-off or to cover for another driver? Was it in 2004?

Jackie Ickx drove for Ferrari twice (1968, and then again 1970-73) after which he had a one off drive for Frank Williams (USA 1973) before re-joining the Frank Williams/Wolf team for the start of 1976.
Andrea de Cesaris had two spells for Alfa Romeo (1980 & 82/83) and two spells for Jordan (91 & two races in 94.)
Of course if we are allowed to include one off drives there must be many others back in the early days - Sterling spings to mind as having two spells at Maserati and three for Rob Walker (punctuated by occasional drives for BRM and a season at Vanwall) and two spells for Vanwall (if you count the one off drive for Rob Walker in Argentina in 1958.)

Maurice Trintignant drove for Ferrari in 1954-55 and 1957, and Rob Walker in 1958-60 and 1962. He also did one-offs in Louis Rosier's car in 1952 and 1954, and for Scuderia Centro-Sud he did one race in 1958, four in 1960, and another one in 1963.

Jose Froilan Gonzalez was with Ferrari in 1951 and 1954, plus one-offs in 1957 and 1960, and with Maserati in 1952-53 and one race in 1956.

Just thought of an interesting one Jean-Pierre Jarier who drove for three teams twice.
In 1974 he joined Shadow where he stayed for basically 3 years before going to ATS in 1977, but he left before the end of the year having one off drives for both Shadow (again) and Ligier. At the start of 1978 he rejoined ATS. Several years and several teams later in 1983 he rejoined Ligier.

Jordan wanted him for France 2002 to temporarily replace one of their drivers but Arrows stopped that. Then Jordan wanted him for 2004 but he said take Heidfeld instead.
Interestingly I read an interview with Eddie Jordan just the other day where he said that in 2001 they were committed to Takuma Sato for 2002 with a bag of cash from Honda I think, and therefore had to ditch one of their drivers. At the time they thought Trulli was their long-term lead driver so they made it known Frentzen would be making way and that's when the relationship crumbled. As it was, they lost Trulli to Renault and suddenly had a vacancy again, and tried to get Frentzen back a few times. Unsurprisingly I think he told them where to go.

Lorenzo Bandini started out in Scuderia Centro Sud's Cooper-Maserati, did a partial programme at Ferrari in '62, before back to Centro Sud (now running a BRM), before re-joining Ferrari mid-1963 for the rest of his career. That's just in terms of F1 WDC races though, think he was on Ferrari's payroll (for sportscar races etc.) consecutively from '62 onwards.

Listened to 2001 French GP qualifying recently a bit, where Brundle said something like that - Frentzen has a nice long-term contract with Jordan, which should take the pressure off him and his long-term future is secured. Well, fast forward a couple of weekends and...
